Please do not fill out this form more than once for the same child. Duplicate forms will be deleted and will not increase your odds of getting a spot.

Children must be 5 years old before September 1 to register for kindergarten. If your child is not 5 years old by the specified date, please wait to submit your application in the year that they will be eligible. We post a new form in January each year. 

Children must be 6 years old before September 1 to register for first grade.
