Student Identification

The Texas Student Data System matches students to their existing school records using one of the following identifiers. You may access your child's S-number or Texas Student Unique Identification on previous school records or by contacting the child's previous school.

Ethnicity and Race Report

This form is to be filled out by the student's parents or guardians, and both questions MUST be answered. Part A asks about the student's ethnicity and Part B asks about the student's race.

The United States Department of Education (USDE) requires all state and local education institutions to collect data on ethnicity and race for students and staff. This information is used for state and federal accountability reporting as well as for reporting to the Office of Civil Rights (OCR) and the Equal Employment Opportunity Commission (EEOC). School district staff and parents or guardians of students enrolling in school are requested to provide this information. If you decline to provide this information, please be aware that the USDE requires school districts to use observer identification as a last resort for collecting the data for federal reporting. PreK Eligibility Determination

The Texas Legislature determines eligibility requirements for free, public PreKindergarten in Texas. When the Texas Legislature established the PreKindergarten program, the intent was, and still is, to provide early learning experiences to students who are most at risk for school failure. Therefore, the eligibility is limited. The legislature believed that a high quality PreKindergarten program could mitigate the impact of the at-risk characteristics, thereby assisting these students to become school ready when they enter Kindergarten. TEC 29.153 (b)

We receive SNAP, TANF or Medicaid. (Please submit your SNAP, TANF or Medicaid Eligibility Letter to the school.)

We are currently homeless. Individuals who lack a fixed, regular, and adequate nighttime residence. The term homeless children and youth includes “children and youths who are sharing the housing of other persons due to loss of housing, economic hardship, or similar reason; are living in motels, hotels, trailer parks, or camping grounds due to the lack of alternative accommodations; are living in emergency or transitional shelters; are abandoned in hospitals; or are awaiting foster care placement.” The term also includes “children and youths who have a primary nighttime residence that is a public or private place not designed for or ordinarily used as a regular sleeping accommodation for human beings,” such as “cars, parks, public spaces, abandoned buildings, substandard housing, bus/train stations, or similar settings.” (Please share circumstances with the school.)

My child is a child of an active duty member of the Armed Forces of the United States including the state military forces or a reserve component of the Armed Forces who is ordered to active duty by proper authority; or who was injured or killed while serving on active duty. (Verification required)

My child is the child of a parent that has received the Star of Texas Award as: a peace officer, a firefighter, or an emergency medical first responder. (Star of Texas Certificate Required)

Household Size

I understand that by submitting this application a verification process is required to determine if my child qualifies for FREE Pre-K. If my child does not meet the requirements for Free Pre-K, I will be notified by the school. If I choose to remain enrolled (based upon availability) I will be invoiced for payment for the cost of the Pre-K program and fees will be determined based upon income.
